Description: 

Home care is provided to patients who need skilled care and care management upon the onset of a medical need. This can include post-hospital or post-surgical care, injury recovery, prenatal and post-partum care, or management of complex conditions like diabetes, congestive heart failure, or COPD.  Many patients who have experienced sepsis need home care. These comprehensive services are provided by nurses, therapists, home health aides, and other direct-care staff. Transitions in care, for example, from hospital to home, require much coordination. For the person recovering or receiving the care and their loved ones, this can sometimes be overwhelming and it can be difficult to determine what types of support are available. What is home care and why is it important for post-sepsis care? In this session, learn how the home care system works, and what you can expect when receiving home care services. You will also learn about possible infection risks while receiving care at home, and steps that patients and caregivers can take to mitigate those risks.
